For health care providers looking to refer a patient , our referral form has been made available below.  You then have the option to either send it to us via email at healthclinic613@gmail.com .
Patients should please note that we do not require a referral from a health care provider to be seen. You are welcome to make an appointment or simply contact us for more information .
